Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor. Sildenafil works by relaxing the smooth muscles in the penis, which in turn enhances blood flow into the corpus cavernosum and causes the smooth muscle cells to grow and relax. It does not protect the blood vessels from being damaged by high levels of nitric oxide (NO) in the corpus cavernosum. In fact, the effect of sildenafil is to increase the level of NO, which can cause the blood vessels to relax. The increased blood flow into the corpus cavernosum can make it easier for the corpus cavernosum to relax and, in turn, to be able to sustain a hard, firm, erect state for up to 6 hours.
